Preparation, taking the test, and understanding the results:

In cases of infertility, evaluation usually begins with the male partner. This is because a sperm test is very easy, painless, and provides the clearest indication of male reproductive potential.

This guide will show you how to prepare for a sperm test and what the results mean.

The most important thing to do before the test: Sexual abstinence.

For an accurate sperm test result, you need to abstain from ejaculation for a certain period of time before the test.

Ideal duration:
Approximately 3–4 days (2–5 days is acceptable)

During this time:

  • Sexual intercourse

  • Masturbation should not be allowed.

Alcohol, smoking, very hot baths, saunas, and strenuous exercise can also temporarily reduce sperm quality.

How to provide a sperm sample?

A sperm sample is usually collected in a laboratory, in a special room, through masturbation, and deposited into a sterile container.

Very important rules:

  • Soap, saliva, cream, lubricant, or condoms should not be used.

  • The entire semen must enter the container. The first few drops, in particular, contain the most valuable sperm.

If you need to bring something from home:

  • It should be transported at body temperature.

  • It must be delivered to the laboratory within 30 minutes at the latest.

What does the doctor look for in the results?

Three main aspects are evaluated in sperm analysis:

1) Is there enough sperm?

The sperm count is checked to see if it's sufficient for pregnancy. A low count reduces the chances of fertilization, but it doesn't make it completely impossible.

2) Are the sperm motile?

For sperm to reach the egg, they need to swim forward. Sperm that are merely vibrating in place are not useful. The doctor checks whether a sufficient number of sperm are actively moving forward.

2) Are the sperm samples morphologically appropriate?

The sperm's head, neck, and tail should be normal. However, what's important here is that a sufficient portion of the sperm, not all of them, are healthy. Because a single healthy sperm is enough for pregnancy. Therefore, the report stating "morphological abnormality" should often not be a cause for panic.

Professor's Note: Sperm production in the body renews itself approximately every 2-3 months. Stress, fever, sleep deprivation, or infection can temporarily impair sperm. Therefore, a single poor sperm test does not definitively diagnose infertility. If necessary, the test should be repeated after a few months.
